Background The expressions of a “wish to hasten death” or “wish to die” raise ethical concerns and challenges. These expressions are related to ethical principles intertwined within the field of medical ethics, particularly in end-of-life care. Although some reviews were conducted about this topic, none of them provides an in-depth analysis of the meanings behind the “wish to hasten death/die” based specifically on the ethical principles of autonomy, dignity, and vulnerability. The aim of this review is to understand if and (...) how the meanings behind the “wish to hasten death/die” relate to and are interpreted in light of ethical principles in palliative care. Methods We conducted a meta-ethnographic review according to the PRISMA guidelines and aligned with Noblit and Hare’s framework. Searches were performed in three databases, Web of Science, PubMed, CINAHL, with no time restrictions. Original qualitative studies exploring the meanings given by patients, family caregivers and healthcare professionals in any context of palliative and end-of-life care were included. A narrative synthesis was undertaken. PROSPERO registration CRD42023360330. Results Out of 893 retrieved articles, 26 were included in the analysis, accounting for the meanings of a total of 2,398 participants. Several factors and meanings associated with the “wish to hasten death” and/or “wish to die” were identified and are mainly of a psychosocial and spiritual nature. The ethical principles of autonomy and dignity were the ones mostly associated with the “wish to hasten death”. Ethical principles were essentially inferred from the content of included articles, although not explicitly stated as bioethical principles. Conclusions This meta-ethnographic review shows a reduced number of qualitative studies on the “wish to hasten death” and/or “wish to die” explicitly stating ethical principles. This suggests a lack of bioethical reflection and reasoning in the empirical end-of-life literature and a lack of embedded ethics in clinical practice. There is a need for healthcare professionals to address these topics compassionately and ethically, taking into account the unique perspectives of patients and family members. More qualitative studies on the meanings behind a wish to hasten death, their ethical contours, ethical reasoning, and implications for clinical practice are needed. (shrink)

Here a wide distribution of meiotic machinery is shown, indicating the occurrence of sexual processes in all major eukaryotic groups, without exceptions, including the putative “asexuals.” Meiotic machinery has evolved from archaeal DNA repair machinery by means of ancestral gene duplications. Sex is very conserved and widespread in eukaryotes, even though its evolutionary importance is still a matter of debate. The main processes in sex are plasmogamy, followed by karyogamy and meiosis. Meiosis is fundamentally a chromosomal process, which implies recombination (...) and ploidy reduction. Several eukaryotic lineages are proposed to be asexual because their sexual processes are never observed, but presumed asexuality correlates with lack of study. The authors stress the complete lack of meiotic proteins in nucleomorphs and their almost complete loss in the fungus Malassezia. Inversely, complete sets of meiotic proteins are present in fungal groups Glomeromycotina, Trichophyton, and Cryptococcus. By tracing some of the historical and hermeneutical influences of Augustine on Martin Heidegger and his 1927 magnum opus, this article argues that Being and Time has an “Augustinian constitution.” While Heidegger’s philosophical terms are in a certain sense original, many of them have their conceptual origins in Augustine’s Christian thought and in his philosophizing from experience. The article systematically revisits all of Heidegger’s citations of Augustine, which reveals not only the rhetorical influence of Augustine on the organization of Being (...) and Time, but also the fact that the conceptual inspiration of the work and the development of its philosophicalterms are significantly indebted to Augustine. Further, an original synthesis of Heidegger’s methodology with Augustine’s thought on restlessness and conversion is developed in order to demonstrate the philosophical compatibility between Heidegger and Augustine. This synthesis results in what the author considers the foundations for an Augustinian phenomenology. (shrink)

The distribution pattern of the meiotic machinery in known eukaryotes is most parsimoniously explained by the hypothesis that all eukaryotes are ancestrally sexual. However, this assumption is questioned by preliminary results, in culture conditions. These suggested that Acanthamoeba, an organism considered to be largely asexual, constitutively expresses meiosis genes nevertheless—at least in the lab. This apparent disconnect between the “meiosis toolkit” and sexual processes in Acanthamoeba led to the conclusion that the eukaryotic ancestor is asexual. In this review, the “meiosis (...) toolkit” is rigorously defended, drawing on numerous research articles. Additionally, the claim of constitutive meiotic gene expression is probed in Acanthamoeba via the same transcriptomics data. The results show that the expression of the meiotic machinery is not constitutive in Acanthamoeba as claimed before. Furthermore, it is argued that this would have no implications for understanding the nature of the eukaryotic ancestor, regardless of the result. (shrink)

Con el objetivo de estudiar algunos problemas de ética relacionados con situaciones clínicas que involucran a la familia, se presenta una revisión sistemática de este término para entenderla y definirla. Concebida como un cuerpo orgánico en su constitución, estructura y organización, en el presente artículo se analizan las variadas circunstancias en las que existe una relación entre la ética clínica y la familia, principalmente haciendo énfasis a la diferencia entre la autonomía individual y la autoridad familiar.

The development of neuropsychological assessment methods using virtual reality (VR) is a valid and promising option for the detection of cognitive impairment in the older people, focusing on activities composed of tasks of multiple demands. This study verified the association of age, schooling, and general cognitive status on the performance of neurologically healthy older adults in ECO-VR, a virtual reality task of multiple demands for neuropsychological assessment. A total of 111 older adults answered a sociodemographic questionnaire, the Mini Mental State (...) Examination, the Vocabulary subtest of the Wechsler Intelligence Scale for Adults (third edition), and the ECO-VR. Correlation analyses, multiple linear regression, and comparisons between groups (effects by age and schooling groups) were used to evaluate the results. The ECO-VR total score was significantly associated with age, years of education, MMSE, and Vocabulary subtest. The linear regression models identified that age was the main predictor for total score and rule breaking of ECO-VR. According to the univariate analysis, it was identified the main effect of age group and schooling group in the total ECO-VR score, but there was no interaction effect. The results are discussed in order to understand the role of sociodemographic characteristics in the performance of older adults in a virtual reality task of multiple demands. It was also verified the possibility use of virtual reality for neuropsychological assessment of older adults. (shrink)

The coronavirus disease pandemic fundamentally disrupted humans’ social life and behavior. Public health measures may have inadvertently impacted how people care for each other. This study investigated prosocial behavior, its association well-being, and predictors of prosocial behavior during the first COVID-19 pandemic lockdown and sought to understand whether region-specific differences exist. Participants from eight regions clustering multiple countries around the world responded to a cross-sectional online-survey investigating the psychological consequences of the first upsurge of lockdowns in spring 2020. Prosocial behavior (...) was reported to occur frequently. Multiple regression analyses showed that prosocial behavior was associated with better well-being consistently across regions. With regard to predictors of prosocial behavior, high levels of perceived social support were most strongly associated with prosocial behavior, followed by high levels of perceived stress, positive affect and psychological flexibility. Sociodemographic and psychosocial predictors of prosocial behavior were similar across regions. (shrink)

Rationale: Deep brain stimulation (DBS) of the hippocampus is proposed for enhancement of memory impaired by injury or disease. Many pre-clinical DBS paradigms can be addressed in epilepsy patients undergoing intracranial monitoring for seizure localization, since they already have electrodes implanted in brain areas of interest. Even though epilepsy is usually not a memory disorder targeted by DBS, the studies can nevertheless model other memory-impacting disorders, such as Traumatic Brain Injury (TBI). Methods: Human patients undergoing Phase II invasive monitoring for (...) intractable epilepsy were implanted with depth electrodes capable of recording neurophysiological signals. Subjects performed a delayed-match-to-sample (DMS) memory task while hippocampal ensembles from CA1 and CA3 cell layers were recorded to estimate a multi-input, multi-output (MIMO) model of CA3-to-CA1 neural encoding and a memory decoding model (MDM) to decode memory information from CA3 and CA1 neuronal signals. After model estimation, subjects again performed the DMS task while either MIMO-based or MDM-based patterned stimulation was delivered to CA1 electrode sites during the encoding phase of the DMS trials. Each subject was sorted (post hoc) by prior experience of repeated and/or mild-to-moderate brain injury (RMBI), TBI, or no history (control) and scored for percentage successful delayed recognition (DR) recall on stimulated vs. non-stimulated DMS trials. The subject’s medical history was unknown to the experimenters until after individual subject memory retention results were scored. Results: When examined compared to control subjects, both TBI and RMBI subjects showed increased memory retention in response to both MIMO and MDM-based hippocampal stimulation. Furthermore, effects of stimulation were also greater in subjects who were evaluated as having pre-existing mild-to-moderate memory impairment. Conclusion: These results show that hippocampal stimulation for memory facilitation was more beneficial for subjects who had previously suffered a brain injury (other than epilepsy), compared to control (epilepsy) subjects who had not suffered a brain injury. This study demonstrates that the epilepsy/intracranial recording model can be extended to test the ability of DBS to restore memory function in subjects who previously suffered a brain injury other than epilepsy, and support further investigation into the beneficial effect of DBS in TBI patients. Review of Kim, Jaegwon. Philosophy of Mind.

The article presents probable impacts from COVID-19 on 5 components of the food system: social organizations; markets; science and technology; nature (biophysical environment); and public policies. It investigates which proposals contained in the State Plan for Food and Nutritional Security of the State of São Paulo (PLANSAN-SP) can respond to these impacts, mitigating them. The impacts of COVID-19 on food systems are broad and profound, considering the different realities raised by bibliographic research. These impacts directly affect the food consumption (...) of the population from the quarantine dynamics adopted worldwide, in various forms, with negative effects on access and availability of food in the short, medium and long terms. PLANSAN-SP contains several proposals to guarantee the Human Right to Adequate Food (DHAA) and to stimulate Food Sovereignty and Food Nutrition and Security (SAN). The article considers that the dialogue between the organized civil society and the public power is fundamental for the construction of proposals adjusted to a diversity of challenges and realities present in the State of São Paulo. It considers also that PLANSAN-SP, if implemented and monitored could respond positively to the impacts of COVID -19 in the São Paulo food system. (shrink)

IntroductionTranscranial magnetic stimulation is a consolidated procedure for the treatment of depression, with several meta-analyses demonstrating its efficacy. Theta-burst stimulation is a modification of TMS with similar efficacy and shorter session duration. The geriatric population has many comorbidities and a high prevalence of depression, but few clinical trials are conducted specifically for this age group. TBS could be an option in this population, offering the advantages of few side effects and no pharmacological interactions. O presente ensaio pretende analisar o Cooperativismo Educacional no estado de São Paulo, em suas formas institucionais, agora voltados para a organização escolar a partir de 1980. O cooperativismo educacional ocupou um lugar de destaque, com um discurso aparentemente renovador, pois tanto pretendia formar o indivíduo integralmente, afirmando superar a ética da competição pela ética da cooperação, de modo a superar a tradição administrativa hierárquica da escola pública e privada e, apresentar, inclusive, supondo desencadear disposições políticas emancipatórias para todos (...) os envolvidos no processo de ensino aprendizagem. Tendo como referencial teórico Dermeval Saviani, na análise da organização da escola e os pressupostos filosóficos e políticos dessa trajetória, e de Antonio Gramsci, ao relacionar as transformações do trabalho e a questão da educação, pretendemos descrever a expansão do cooperativismo educacional e analisar os seus princípios pedagógicos, organizacionais e políticos. This essay aims to analyze the Cooperative Education in São Paulo in its institutional forms, now facing the school organization since 1980. The cooperative education occupied a prominent place, with a refreshing speech apparently because both wanted to form the whole person, claiming to overcome the ethics of competition for the ethics of cooperation, in order to overcome the administrative tradition of hierarchical public and private schools, and provide even emancipatory political arrangements for all involved in teaching and learning. The theoretical framework Dermeval Saviani, the analysis of school organization and the philosophical and political assumptions that trajectory, and Antonio Gramsci, to relate changes in work and the issue of education, we intend to describe the expansion of cooperative education and analyze the pedagogical principles , organizational and political. Para o efeito, analisa-se, primeiramente, a argumentação segundo a qual, sob certas condições lógicas, teológicas, ontológicas e cosmológicas antinecessitantes, detetadas por G. W. Leibniz, a abertura contingente do futuro parece ser compatível com o regime das "verdades contingentes pré-determinadas", regime enquadrado teologicamente pelo princípio do "futuro melhor" ou do "único futuro verdadeiro". No entanto, os futuros contingentes incitam, com e contra Aristóteles, ao desenvolvimento de uma lógica temporal e plurivalente, (...) ao modo de J. Łukasiewicz ou A. Prior. Essa lógica garante a abertura do futuro sem o oneroso custo metafísico da adesão a uma teo-lógica omnideterminante. A crítica do determinismo lógico, daí resultante, afigura-se mais coadunável com as condições pós-metafísicas inerentes à episteme agnósticacontemporânea, mas, nesse caso, a abertura do futuro implicaria uma profunda redefinição das próprias ideias e funções de "Deus", "matéria", "história" e "verdade". ABSTRACT: This paper investigates the theological essence of contingent futures. First, it analyses the argument according to which, under certain logical, theological, ontological, and cosmological conditions detected by Leibniz, the contingent openness of the future may be compatible with the regime of "pre- determined contingent truths", theologically grounded in the principle of an "optimal future" or a "single true future". However, the examination of contingent futures has also fostered, both for and against Aristotle's position, the development of a many-valued and temporal logic as proposed by J. Łukasiewicz or A. Prior. This logic safeguards the openness of future without the metaphysically expensive cost of adhering to an all-determining theo-logic. It hence follows that the critique of logical determinism seems to be more appropriate to express the post-metaphysical conditions of our present agnostic mentality. In this case, however, the openness of the future would entail a profound redefinition of the very ideas and functions of "God", "matter", "history", and "truth". (shrink)

Automatized scalable healthcare support solutions allow real-time 24/7 health monitoring of patients, prioritizing medical treatment according to health conditions, reducing medical appointments in clinics and hospitals, and enabling easy exchange of information among healthcare professionals. With recent health safety guidelines due to the COVID-19 pandemic, protecting the elderly has become imperative. However, state-of-the-art health wearable device platforms present limitations in hardware, parameter estimation algorithms, and software architecture. This paper proposes a complete framework for health systems composed of multi-sensor wearable health (...) devices, high-resolution parameter estimation, and real-time monitoring applications. The framework is appropriate for real-time monitoring of elderly patients' health without physical contact with healthcare professionals, maintaining safety standards. The hardware includes sensors for monitoring steps, pulse oximetry, heart rate, and temperature using low-power wireless communication. In terms of parameter estimation, the embedded circuit uses high-resolution signal processing algorithms that result in an improved measure of the HR. The proposed high-resolution signal processing-based approach outperforms state-of-the-art HR estimation measurements using the photoplethysmography sensor. (shrink)

Background Infertility is an increasingly prevalent disease in society and is considered by the World Health Organization to be a public health problem. An important ethical issue arises from the clarification of reproductive rights in a fair and equal way. The objective of this study was to deepen and update the knowledge and discussion about the difficulty of accessing infertility treatments in Brazil. Methods A cross-sectional observational study was carried out through the application of an online questionnaire that collected the (...) socioeconomic characteristics of couples and identify how barriers to infertility care affect the most vulnerable populations. We included couples who sought medical assistance to achieve pregnancy at two clinics in the states of São Paulo and Minas Gerais. Results A total of 201 questionnaires were analyzed. Most couples self-declared as white and the average age of wives was 36 years and husbands 38 years. 65% (65%) of couples would proceed with the treatment in a different city to which they lived, 37% evaluated as having easy access to a medical specialist only after indication, and more than half of the participating have thought about giving up the treatment due to some difficulty in accessing it. 39% of participants sought more than one medical service to find better reception, 42% of couples sought more than one medical service to define where it would be better financially, and 67.2% referred to the high cost of treatments, that is, financial issues, as a great difficulty in accessing medical services and/or treatment. Although 72.6% of couples considered having a good quality of life, 54.2% admitted that infertility and the search for treatment generated anxiety/stress in the couple’s life. Conclusion There is a need for public education on reproductive health and for policymakers to raise awareness of the importance of the difficulty that many couples face in seeking treatment to become pregnant, especially in countries with less financial resources. Indeed, it is commonly accepted that there is a universal human right to access healthcare of appropriate quality as a matter of justice. Discussion of access to reproductive technologies should be considered taking into account the longstanding ethical debate regarding fertility, fecundity, and infertility, as well as reproductive care.
